What do you use for extremely dry skin?

I have this problem every winter. My skin gets extremely dry, especially my shins, feet, hands, and heels. Well, probably almost every where really.

But this year it has gotten so bad, I am scratching myself raw and making my skin bleed.

What do you all use for dry, itchy skin this time of year?
