There are several blood tests you can take to see if you do have PCOS. I have never heard of having to have surgery to find if you have PCOS. Most women who have PCOS usually have irregular or absent periods (usually 6 or fewer a year) which causes you to NOT ovulate, have a high testosterone level, their FSH/LH (I think that's the combo) is irregular, male pattern baldness, male pattern hair growth (face, stomache, back, chest), adult acne, skin tags, and other symptoms.
I was diagnosed by answering a few questions from my OBGYN (back in 1995), it was then confirmed blood work (back in 2002).
I see some actual PCOS symptoms that you've posted, some of the others you've mentioned I've not seen as being a PCOS symptom myself, but I'm not a medical professional so they could be. Each person is completely different when it comes to symptoms.
I would definately bring PCOS up to your doctor at your appointment and see what they say. Explain to them your symptoms and how you feel that PCOS could be the factor and that you'd like further examinations.
