Default Please Help...New to PCOS

On Feb. 16 I had my second first trimester m/c and decided to see a RE. So far my blood work has come back normal. I have regular cycles (on and off b/c) and I ovulate regularly...because I have no problems getting pregnant. I went off bc Dec. 1 and had my first period Dec. 31 and got pregnant on that cycle. I did a fasting blood work and my glucose came back normal, which I've never had a problem with that. My insulin however (didn't know there was a difference) came back as 42. My RE called and started me on Metformin. I was shocked! The nurse said I probably have PCOS because of the high insulin and that could be a cause for my m/c. I am producing bad eggs and not having a viable pregnancy. We are still waiting on the chromosome results (takes 4-6 weeks) and HSG to see if I have any scar tissue from the D&C I had due to my first loss. What is PCOS? Can you have it and have normal periods and ovulate normal? I've tried to read about it and the only thing I can find that I have symptoms of it is the insulin resistance and I have gained weight and carrying it in the middle. But, I haven't gained but maybe 2lbs a year, but it does seem hard to lose it. I was told to take Metformin for 6 weeks and April 16 they will test me again. I take 500mg (first week take one pill with evening meal; second week take two pills with evening meal; third week take three pills with evening meal). Is this going to put a hold on trying to get pregnant again? My RE, before the insulin thing came up, said if nothing showed on chromosomes and all the blood work came back we could try again. This is all new to me and I'd appreciate any help I can get.

First, I am sorry for your loss.

Second, Metformin will not keep you from trying for another pregnancy, as a matter of fact many doctors believe that it reduces miscarriage rates. Your insulin was very high so it is a good idea (IMHO) to take the Met.

There are many women with PCOS who have "normal" cycles and are not overweight. There are lots of articles out there that you can read that may really be helpful. PCOS is a common issue and is very treatable so don't let it freak you out.

Make sure you take the Met with food as it can make you queasy and it can give you loose stools as well, so talk to your doc if you have problems.

I have been diagnosed with PCOS for years and been taking Met off and on for quite a while so if you have specific questions feel free to ask me.

Thank you so much for writing. When my RE told me I may have PCOS it did freak me out because I've read where it's so hard to get pregnant and it's something that isn't curable, but treatable. If it means taking a pill everyday to have a baby, then I'm all for it!

You may not have PCOS exactly, but an insulin level of 42 indicates insulin resistance, which can lead to diabetes and a high rate of miscarriage. Many women with IR also present with PCOS, but not definitively. PCOS is poly-cystic ovaries, and it is really the name for a group of symptoms. There is no one standard for how many symptoms you need and to what degree for doctors to call it PCOS. One of the "classic hallmark" signs of PCOS is too many follicles that are quite small and bunched together (a string of pearls) when viewed on an u/s (usually around day 3). I have PCOS - I ovulate regularly (at least while taking metformin) but I have typical "polycystic" looking ovaries. Most of my bloodwork is normal, but I have borderline high insulin levels. I was diagnosed first by my symptoms, and then second because of the IR. I take metformin. People with PCOS/IR are about 65% likely to m/c if not treated, but the rate of m/c drops to "normal" rates once on a medication like metformin.
