Default Night & Daytime Sleep Q's - Help!

My baby is 4 months old. At nighttime we have a ritual of rice cereal, bathtime, brush teeth, put on Baby Einstein music, nurse on one side. He almost always passes out on my breast and I put him down and he is out for 5-8 hours. Is this bad that he falls asleep on my breast? Do I need to stop his feeding while he is still awake? If he doesn't fall asleep on my breast we let him CIO and he usually falls asleep within 30 minutes to 1 hour of crying.

During the day his sleep is horrible! I don't actually put him down 2 hours after waking or 2 hours after every nap which could be the problem. I tried that and I felt like it was just taking too long trying to get him to sleep. Like today for example he woke up at 7AM. I went to an exercise class that started at 9AM and he fell asleep in the car after class at 10:30AM. He slept until 11:30AM in his carseat. Then he got fussy at 2:30PM so I tried to put him down. I let him CIO for 1 hour and then went and picked him up. I just put him to sleep at 6:30PM and he went down easy.

Any suggestions? I get so stressed everyday because I just want a happy, rested baby but I can't let him CIO so much because I just want to play with him. Do I need to go through a few days of hard core training so I can get him into a good schedule?
