Ugh, how can you refuse such a cute pouting face when they want something so bad???
At the moment, I agree with Mary Beth on the whole timing issue. I wouldn't want to take that task on right now with you in so much pain. But.....
OK, you asked for advice for getting rid of the bottle, here is what I would do. Stop making bottles. Seriously. He knows how to drink out of a sippy cup, right??? Yes, I am sure he can so I would not be making bottles and having them on hand. I know, I know, it's going to be rough, but if you really are sick of them and ready to be done, you have to take charge of the little man.
No more bottles before naps. Offer the sippy of milk instead. Since you don't hold him now while he is drinking his bottle, I suppose I would just do the same routine, but hand him the sippy instead, saying, no more baba, here is your milk. Let him drink from the cup without the cap on first, so he knows it's milk in there. That seemed to work for us in the very beginning.
When you are through with no bottles before naps, (and really, only 2 days of this was all it took for us....I thought it would be so much longer) you can tackle the night time bottle. I tried to do the same night routine, sitting in the rocking chair, holding him and listening to his music box with a bottle. SCREAMING took place after 2 nights of this!!! I went to the kitchen, got a sippy of milk instead and still had screaming. The 3rd night I just went right for the sippy cup and that worked for some strange and great reason.
I packed away all bottles and never looked back. So 3 days broke the habit here. You just have to be EXTRA diligent. No daytime bottles for 2 days, no nighttime bottles after 2 nights. (and he didn't drink the bottle the 2nd night when I offered it)
Be strong, mama, he will (eventually

) drink his milk from a sippy when that is his only choice.
